| Grade Check Dates 2025-2026 |
| Students participating in activities will have their grades checked roughly every three weeks to determine their eligibility. |

| WIAA Physical Form |
| Students participating in athletics must receive a physical every 2 years. This form must be signed by a medical professional and return to the AD in order to be eligible to both practice and compete. |
|
| WIAA Alternate Year Physical Card |
| On the alternate year when a physical is not required, a parent or guardian is required to complete this form. Students are not eligible to both practice or compete until this is completed and returned to the AD. |
